Access to Medical Records and Private Medical Forms

The Tredegar Practice Medical Notes and Medical Form Completion Procedure

We are a Practice who is aspiring to reduce our wastage to benefit the environment. For this we are implementing new measures to help us aid our goals by reducing paper waste. We have taken the step to minimise our paper wastage when we are providing access to medical notes to patients by putting these steps in place.

  • Your request will be logged onto our central database

 

  • Your request will be logged onto our central database

 

  • Once logged we will contact patient and will set up patient with online access to their FULL medical record. (Exemption will be made on a case by case basis)
  • Request will be forwarded to practice administrator.

 

  • Once online access is set up patient will be able to share or send their recird to third party of their choice via email or post.
  • An invoice will be sent and on completion of payment your form will be completed.
  • Once completed we will post using Royal Mail Signed For 1st Class and will email postage reference number on the email provided on the surgery request form.
For any queries and updates regarding an existing request please complete Medical Notes, Medical Form/Questionnaire Request Form and email to the Tredegar Practice and we will be in contact with you as soon as possible

 

  • Benefits of setting up online access are that it will always be the up to date version of your medical record.
  • Once set up you will not need to contact us again if you need to share your records with someone else
  • You will have a variety of ways of sharing your record either electronically, via email or via post which are not always available to the practice.

 

Access to medical records

The practice is registered and complies with the Data Protection Act 1998. Any request for access to notes by a patient, patient’s representative or outside body will be dealt with in accordance with the Act. Please contact the Practice Manager for further information.

Your Data Matters to the NHS

Your health records contain a type of data called confidential patient information. This data can be used to help with research and planning.

You can choose to stop your confidential patient information being used for research and planning. You can also make a choice for someone else like your children under the age of 13.

Your choice will only apply to the health and care system in England. This does not apply to health or care services accessed in Scotland, Wales or Northern Ireland.
